Community Participation

Every person has their own dreams and ambitions, abilities and areas of need. Lifeskills Plus supports people to achieve their own goals in life.

When you chose Lifeskills’ Community Participation Service you will have access to support in many areas to help you increase your daily living skills, social and recreational abilities.

You will have an individual plan that is developed with yourself, your family /important people in your life and Lifeskills Plus Inc.

The things that are put in a PCP (Person Centered Plan) are things that are important to you and things that can help you achieve your goals.

Programs are developed according to your current skills and abilities and are adjusted with you as your levels of ability increase.

Eligibility

To be eligible for Community Participation Service you must have a current CP package from ADHC

Transport

Transport may be provided during the day to assist a person to access program activities, but it is not provided for commuting purposes at the beginning or end of a respite session - other than in limited & agreed circumstances.
